Any person filing an application for issuance or renewal of any business license shall include in the application the following information:

A. The exact nature or kind of business for which a license is requested and the goods to be sold;

B. The name, home and business address of the applicant, and the name and address of the owner, if other than the applicant, of the business; the social security number and birthdate of the applicant;

C. The applicant shall present a current driver’s license or other identification;

D. The proposed location of the business, event or sale;

E. A statement or lease agreement from the owner granting the use of the property for conducting the business, event, or sale;

F. The dates of the proposed event or sale and the length of time for which the right to conduct the sale is being requested;

G. The time of operation during each business day of the proposed event or sale;

H. The applicant’s current taxpayer identification number or evidence of application for a current taxpayer identification number;

I. A copy of the license any state or other authority required for the business to be conducted; and

J. Such further information as the clerk may deem necessary to enable him/her to issue the license. (Ord. 1337-88 § 3(part), 1988).
